How information is handled

Access is limited to authorised Polaris staff and service providers that operate this secure form. Form answers and issued versions are encrypted at application level. Email messages contain secure links only, not form answers or PDF attachments.

Source-tagged proposals

Some answers may be proposed from documents already available to your adviser in the relevant ChatGPT Project. The form stores only the proposed value and a minimal source label such as filename, page, and confidence. It does not copy those source documents. Proposed values must be reviewed before they are accepted.

Retention

Form payloads, versions, and generated PDFs are scheduled for deletion 90 days after confirmation or last relevant activity. A non-personal deletion record may be retained to show that deletion occurred.

Your choices

You may request a correction instead of confirming an issued version. Contact your Polaris adviser to ask for access, correction, revocation of a link, or more information about handling your personal information.

Approval required: Polaris must confirm its privacy officer contact, legal bases, overseas disclosure wording, final retention trigger, and declaration text before production use.
